It has been an honor and privilege to serve you and your family's eye care needs for the past 30 years. I am incredibly grateful for the trust you have placed in me and the warm relationships we have built over the years.
Effective June 1, 2025, I am retiring from my private practice. To request a copy of your last eyeglass, contact lens prescription, or medical record, send an email to:
valleyeyecareassociates@gmail.com. In the body of the email, please include your full name and date of birth.
As a reminder, eyeglass prescriptions expire 2 years after the date of the eye exam and contact lens prescriptions expire after 1 year. If your prescription has expired, it will be noted as such.
